Dear Sirs,
The following documents are necessary in order to submit visa application:
 
Filled in and personally signed visa application form;
Invitation letter from Voivodeship Office or
Certificate or voucher from a travel agency; 
Copy of the passport, the page with the personal data;
Bank statement for the last three mounts;
Working agreement;
One picture en face;
Travel insurance valid in all countries of  EU, covering the period 
of the validity of the requested visa;
 We inform that visa applications can be submitted from Tuesday to Friday, from 10.00 to 13.00, and there is no need for previous appointment.  The standard visa procedure lasts up to 10 calendar days and the fee for processing visa application is 3672 DEN. 
Polish Schengen visa for holders of RKS passport is not valid only in Slovakia, Spain, Portugal, Malta and Greece
